ILLINOIS RELIGIOUS FREEDOM PROTECTION AND CIVIL UNION ACT
Effective June 1, 2011

This notice is to advise you of the Illinois Religious Freedom Protection and Civil Union Act (the Act), and its impact on your insurance coverage.

The Act creates a legal relationship between two persons of the same or opposite sex who form a civil union pursuant to the Act. A marriage between persons of the same sex, a civil union, or a substantially similar legal relationship other than common law marriage, legally entered into outside of Illinois, will be recognized in Illinois as a civil union.

Parties to a civil union are entitled to the same legal obligations, responsibilities, protections and benefits that are afforded or recognized by the laws of Illinois to spouses.

Effective June 1, 2011, the term 'spouse' in policies, certificates, and applications will be considered to include the parties to a civil union. Policies of insurance that provide coverage for children will include coverage for the children of civil unions.

The term "spouse" means a legally married man and woman.

The term "Domestic Partner" or "Civil Union Partner" means two people of the same or opposite sex who have entered into a personal relationship or union that is recognized by a state or government without the implicit historical and religious meaning associated with the term "marriage." A "domestic partner" or "civil union partner" does not include:

persons younger than 18 years of age;
persons who enter into a personal relationship or union prior to the dissolution of a legal marriage, previous personal relationship or union, or substantially similar legal relationship of one of the parties;
persons who are ancestors, descendents, or siblings whether the relationship is by the half or the whole blood or by adoption;
aunts or uncles and a niece or nephew, whether the relationship is by the half or the whole blood or by adoption; nor
first cousins.

Replacement Coverage

NOTICE REGARDING REPLACEMENT
REPLACING YOUR LIFE INSURANCE POLICY OR ANNUITY

Replacement Coverage: What is a "Replacement?" A replacement occurs when the applicant cancels, lapses or surrenders an existing policy or certificate of coverage to purchase a new contract with an existing or new insurance company. It can also occur if the applicant uses the cash value from an existing policy or certificate to pay for new coverage.

Unless you are canceling, lapsing, or surrendering your current coverage, or using cash value from an existing life insurance certificate of coverage to purchase new coverage, you should answer NO to this question.

Automatic Premium Loan Provision

The "Automatic Premium Loan" provision will be in effect when "yes" is selected. Automatic premium loans allow unpaid premium to be automatically deducted from the cash surrender value, if any, to prevent lapses in policy coverage.

While this provision is in effect, if the cash surrender value is sufficient, we will automatically make a loan at the end of the grace period to pay any unpaid premium. This loan will cover the amount owed for the billing period last paid.

If the cash surrender value is insufficient to pay the unpaid premium, we will change the premium frequency, if possible, so that a smaller premium is due. If we cannot change the premium frequency, or if once changed, the cash surrender value is still insufficient to pay the premium, no loan will be made and the "Policy Values" section will apply.

Any loan created as a result of this provision is subject to the terms and conditions of the "Policy Loans" provision.

Hospital Accident Care Plan

Our Hospital Accident Care Plan pays certain benefits related to hospitalization after an accident. All benefits are subject to the coverage certificate's schedule, limitations, terms and exclusions.

Hospital Daily Benefits Plan

Our Hospital Daily Benefits Plan is available to members and benefits start on the very FIRST DAY of your hospitalization. Other plans may not cover all expenses for a confinement in a hospital, skilled nursing facility, intensive care unit or for licensed home health care, leaving you with unpaid deductibles, co-payments and other unexpected charges. The Hospital Daily Benefits Plan can help fill the gap between the total cost of a hospital stay and any benefits you may receive from other coverage.

At ages 65 and older, the benefit will be 50% of the Daily Indemnity benefit for the first 60 days of continuous Hospital Confinement. Thereafter, the benefit is 100% of the Daily Indemnity benefit for the rest of that Hospital Confinement.

When Member & Family coverage is selected: Spouse benefits match Daily Indemnity benefits schedule. Children's benefits are 60% of Daily Indemnity benefits.

All benefits are subject to the coverage certificate's schedule, limitations, terms and exclusions.
